Review of Late Certification
This form is required when the employee certified their effort in Oracle after the semi-annual period close date. This form is utilized only when the effort listed on the last Oracle generated Effort Report is an accurate representation of the employees’ work effort for the semi-annual period. If the effort listed on the last Oracle generated Effort Report is NOT an accurate representation of the employees’ work effort for the semi-annual period, a Recertification of Effort form is required.
It is important to provide complete and correct information; partial or incomplete responses will be returned to the departmental contact listed on the form. Once approved, the non-compliance state is removed from the employees’ Oracle Personnel Record.
