Nonstandard Husbandry Form (login required) - required when part or all of the husbandry for a group of animals will be performed by the research staff (e.g., special diet, treated water, timed feeding, water/food measures, etc.) Instructions for use of the Nonstandard Husbandry Form
Human Patient Area Request required when animal procedures occur in a space designated for human patients. This completed and signed form must be uploaded to the protocol form (Species Information, Page A, Animal Use and Procedure Areas).
eReports
E-Reports are web-based generators that researchers, staff, and administrative offices can use to query data, find uploaded documents, and create standardized forms from the IRAP database. All IACUC eReports noted below can be found on the Office of Research eReports (login required) under the IACUC tab.

Adverse Condition Alert Signage
Purpose: To alert care staff, veterinarians, and inspectors to observable characteristics related to the approved study. As an example, a study that involves skin cancer may include a certain level of skin lesioning. Criteria for endpoints and possibly pictures of normal versus endpoint lesions could be included to alert staff of what to expect in the experimental animals.
Instructions
- Enter the numerical portion of the animal protocol number (APN) in the field and hit enter.
- Select the appropriate species in the Species menu (only the species approved under this protocol will appear as choices).
- Click the “View Report” button. A preview of the report will appear.
- Click the down arrow next the Save icon
and select the format you would like to use to print/save the signage.
NOTE: If you need to amend the information that is already included in the report or need to add pictures select the Word option to edit the report information.

Animal Use Safety Information (AUSI)
Purpose: This report is used exclusively by EH&S staff members to create signage for animal rooms where hazardous agents are in use. DO NOT use this report. All official AUSIs must come from EH&S and through the ARP Health and Safety Specialist.

Correspondence from IACUC Office Report
Purpose: To provide access to all emails and attachments sent through the IRAP system to PIs and designated contacts. A common example of use would be obtaining the most recent approval letter for a protocol for use with progress reports or student candidacy/thesis/dissertation forms.
Instructions
- Enter the numerical portion of the animal protocol number (APN) in the field and hit enter.
- Click the “View Report” button.
- The results display in newest to oldest order. Use the Email Subject Line and/or Email Attachment columns to find the correspondence needed. For example, if you need the most recent approval letter look for the most recent attachment titled Approval Letter or Approval Letter DOC.

LA Protocols by Investigator
Purpose: To provide a full list of active protocols their next review date for a specified PI. Includes inactive records as well. This report can be helpful if you would like to know when protocols will require action. For example, if you know you will be away for an extended time or may be busy with grant writing, study section, etc. you can see in one place whether your current protocol renewals will overlap with that activity.
NOTE: You will only be able to see information for PIs/protocols to which you have been granted access.
Instructions
- Enter the last name of the PI in the Investigator field and hit enter.
- Select the correct name from the Investigator drop down menu.
- Click the “View Report” button.
The resulting report will be broken into 3 sections:
- i. Active Protocols Due for Review within 90 days
- ii. Other Active Protocols
- iii. Protocols Under Development (will not display if no protocols under development)
- iv. Inactive Protocols

Nonstandard Husbandry
Purpose: Required signage when a protocol is approved with 1) animal husbandry performed by the lab staff, and/or 2) animal husbandry performed outside of the established ARP standards (e.g., special diet, water regulation, diabetic water and cage management, etc.)

- If needed, select the corresponding species.
- Click “View Report” button. A preview of the report will appear.
- Click the down arrow next the Save icon
and select the PDF option to print.
- Additional contact information and the Month/Year fields can be handwritten if needed, but the Description field should only include the items already approved and populated (see NOTE above).

Purpose: This report is for IACUC Office and OSP Office use

Student Candidacy IACUC Approval
Purpose: To provide documentation for graduate students confirming their inclusion on an approved IACUC protocol.

- Click the “View Report” button. A preview of the report will appear.
- Click the down arrow next the Save icon
and select the PDF option to print.
