The Verification Request is for former students and/or graduates of the UAB School of Nursing. Current students should contact sonregistrar@uab.edu for requests.

Transcript requests need to be made through https://www.uab.edu/students/one-stop/grades/academic-transcripts

If you submit any attachments/paperwork, be sure to complete any portions of the forms requiring your completion and/or signature before uploading.

Upon submitting this form you will be taken to our payment processing site Touchnet and your verification request will not be submitted until your payment has been processed.

Please keep in mind that if you need to submit multiple verification requests, you must submit each request separately.

If you have already submitted a request, do not submit a duplicate request, as once payment has been made there are no refunds. Instead contact sonregistrar@uab.edu with any questions or concerns you may have about your submitted request.
