Employee Injury

If you have an injury requiring medical treatment that would cause a bill to be generated, you must complete an incident report. Campus employees will use the On-the-Job Injury and Illness Reporting form. Medicine Employees will use Trend Tracker. Contact your supervisor for further questions.

The On-The-Job Injury & Illness Program (OJI) is responsible for coordination of benefits payable, including medical expenses and lost wages, to any employee who may have an injury and/or illness sustained in the course of, and arising out of, employment at UAB.

Transporting Injured Employees

After contacting Employee Health Needle Stick and Exposure Team and being assessed by Employee Health staff, and if it is deemed necessary to visit The WorkPlace, injured employees should use Blazer Express for transportation to and from The WorkPlace for their initial visit. The WorkPlace is located at UAB Highlands, Suite 100, and the phone number is 205-933-5300.

After-Hours Reporting

If an incident occurs on the weekend or outside normal business hours, employees should contact the Employee Health Needle Stick and Exposure Team by calling 205-934-3411 an asking for “the Needle Stick and Exposure Team on call”.
